Man, I’ve been overthinking what I myself would want to suggest, but I think I have an idea.
What about some kind of fantasy alien? Could be related to whatever alien-equivalents already exist in D&D (like I think illithids?), but either way I’ve always liked mixing fantasy and sci-fi.
Uh, that's a really interesting idea. There are star spawned creature and, as you said, illithid and the like, but nothing too like an actual alien. Or at least nothing official that comes to mind.
